Vaccinations

What about vaccinations/immunizations exemption?

Iowa has a proof of immunization requirement to enroll in a school. This requirement also affects those choosing to homeschool under the CPI law. If you choose to vaccinate that information should be submitted with your CPI Form.

Medical Exemption

A Medical Exemption may be granted if your healthcare provider believes immunization(s) would be harmful to your child or someone in your house. The Iowa Department of Public Health, Certificate of Immunization Exemption must be signed by a physician (MD, DO), nurse practitioner, or physician assistant to be valid. The Certificate of Immunization Exemption should be submitted along with your CPI Form.

Religious Exemption
If immunizations conflict with a religious belief and is not based on scientific, medical or personal opinion, an Iowa Department of Public Health, Certificate of Immunization Exemption may be completed. To be valid, the religious exemption must be signed by the individual or the child’s parent/guardian, and notarized. The Certificate of Immunization Exemption should be submitted along with your CPI form.
